Location: Covering Denbighshire (on-site)

Salary: £25,541 to £27,200 per annum, pro rata

Hours: 35 per week

Contract: Fixed-term contract (until 31 March 2027)

Additional Requirement: Candidates must hold a full driving licence (held for 2 years) and have access to a vehicle.

Do you have knowledge of the services (statutory and third sector) available in the Denbighshire community? If so, this role could be for you!

Our team of Community Navigators in Denbighshire provides a link between Health and Social Care professionals, Denbighshire citizens, their families, and their carers. As part of the team, you would provide support within the community and the third sector. This approach can also be called 'Social Prescribing', which is a mechanism for connecting citizens with non-medical sources of support in the community. You will recognise that people are experts in their own lives, but sometimes they need someone else to help them identify and work towards the outcomes they would like to achieve.

What will a day in the life of a Community Navigator involve?

• You will work closely with Community Resource Teams, 'Talking Points', SpoA Wellbeing Coordinators, the Complex Disabilities Team, Primary Care, Community Mental Health Teams, and other individuals from a variety of local services.

• You will engage with 'Talking Points' service users in their usual homes or perhaps in another location. You will conduct needs assessments and develop support plans using the "What Matters" approach.

• You will be a source of current, accurate, and timely information about a comprehensive range of support available in the community. You will improve the quality of the service for the benefit of service users.

What do you need to be a successful Community Navigator?

• Be proactive, able to respond quickly to requests for service, and understand the importance of the service you provide.
• Deal with enquiries in a diplomatic, confidential, and non-judgmental manner.
• Feel comfortable planning your own workload and working remotely while still being part of a team.
• Excellent communication skills, both written and verbal.
• IT proficiency, especially with Microsoft 365 packages.
